Is Your Overactive Bladder Treatment Actually Working? Meet eCoin

If you’re tired of taking medication after medication with minimal results, or frustrated with treatments that require weekly appointments, then it’s time to talk about tibial nerve stimulation. eCoin is one such stimulator.

As a pelvic surgeon and advocate for women’s bladder health, I’ve had the chance to work with many treatments for overactive bladder (OAB).I am a big fan of the tibial nerve stimulators and I have the most experience with ecoin And I have to say, this small, nickel-sized device is one of the most promising options I’ve seen for long-term symptom relief without the ongoing burden of medications or office visits.

What Is the eCoin?

eCoin is a tibial nerve stimulation device that’s implanted near the ankle. It delivers gentle pulses to help regulate bladder activity, similar to how Botox or sacral nerve stimulation works, but in a much more streamlined way. Once implanted, it quietly does its job, no weekly appointments or complex adjustments needed.

Why Simplicity Matters

One of the biggest barriers to effective OAB treatment is consistency. Many women simply can’t manage ongoing office visits or tolerate the side effects of medication. eCoin was developed with that in mind. It’s a “set it and forget it” approach, providing reliable stimulation for 3–6+ years on average before needing a battery replacement.

Even better? Battery replacements can often be done quickly and comfortably in the office.

How Well Does It Work?

According to clinical studies:

  • 75% of women experienced a reduction in OAB symptoms by at least 50%
  • 25% reported complete relief of symptoms
  • 89% said they would recommend eCoin to a friend

Those are powerful numbers, especially considering how many women with OAB feel like they’ve already “tried everything.”

No More Weekly Visits, No More Guesswork

Unlike Botox or PTNS (percutaneous tibial nerve stimulation), eCoin doesn’t require repeated injections or weekly maintenance sessions. After a short healing period, it goes to work quietly and effectively. And with a systematic placement process that doesn’t require intraoperative testing or external programming, it’s designed to be uncomplicated for both doctors and patients.  It does need to be replaced when the battery dies which is every 3-5 years right now.  The battery technology should continue to improve.

Who Is eCoin Best For?

If you:

  • Still feel urgency, frequency, or leaking despite trying medications
  • Want a long-term solution without invasive surgery
  • Need something that works with your schedule, not against it
  • Feel like your bladder symptoms are limiting your quality of life

Then it might be time to talk to your provider about eCoin. Many women with mild to moderate overactive bladder are good candidates.

From My Perspective as a Pelvic Surgeon

Operating on the ankle might sound intimidating, but eCoin is refreshingly simple. The procedure can often be done in a surgery center or office setting under local anesthesia. Recovery is fast, and most women return to daily activities quickly.

And what I find most meaningful? The look of relief on a woman’s face when she realizes she doesn’t have to plan her life around bathroom access anymore.

Let’s Keep the Conversation Going

Overactive bladder doesn’t just affect your bladder—it affects your confidence, your freedom, and your joy. You don’t have to live with constant urgency or worry.

eCoin is one of the many tools we now have to treat OAB effectively and simply. If you’ve been stuck in a frustrating cycle of appointments and side effects, know that there’s hope.
